About this template

Most application forms outside recruiting share a shape: a routing question, a short justification, supporting documents, contact details. Awards programs, membership applications, scholarship submissions, accelerator intakes, and partner-program signups all fit. This template ships the shell. Configure once and reuse across the year's applications without rebuilding.

The first block routes by application type. An award program might have Individual / Team / Organization. A scholarship program might have Undergraduate / Graduate / Continuing. A partner program might have Reseller / Integration / Co-marketing. Each branch unlocks the right follow-up fields. Logic is free on every plan, so branching doesn't migrate to a paid tier as the program scales.

The supporting-document block is a file upload. Common requirements: CV, project plan, letter of recommendation, portfolio. AntForms uploads work without forcing a respondent sign-in, which matters for international applicants and any program where the applicant isn't already a customer. The 25 MB default covers most materials; raise it on Pro for video portfolios.

Application forms break on the volume curve. A scholarship program announced on social media, an award program profiled in a publication, or a partner program covered by a newsletter can produce a 50× submission spike over 48 hours. AntForms has no cap on any plan, so the spike doesn't become a billing event or cap-induced data loss. The dashboard scales to thousands of submissions; CSV exports handle large batches.

Yes. This shell covers both. If you only need a hiring form, the dedicated /templates/job-application template has more job-specific fields. Pick this one when the same form needs to handle multiple kinds of application across the year.

Edit the first multiple-choice block and replace the placeholder options with your program names. Then on each next block, set a "show only if" rule referencing the application type you want to surface that block for.

Yes. AntForms has no submission cap on any plan. A 1,000-submission day costs the same as a 10-submission day. The dashboard, exports, and integrations all scale. Review teams usually CSV-export and use a tagging column to triage large batches.

On Pro, yes. The form supports save-and-resume and respondent-edit links. On Free, each submission is final once submitted, but you can include a "fix something? email us" line on the thank-you screen with a mailto link.
